Initial Comments:
An off-site follow-up survey completed on July 15, 2016, found that Freedom At Home, LLC had corrected the deficiencies cited under 28 PA Code, Part IV, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. These deficiencies were cited as a result of a State re-licensure survey conducted on May 2, 2016.
